Here you can find information regarding accreditation and accommodation during the 2004 IIHF World Championship to be held in Prague and Ostrava.

The deadline for media accreditation applications is January 16, 2004.  Applications must be approved by the Organizing Committee, the IIHF, and Infront Sports & Media.  All applicants will be informed of accreditation status, by March 1, 2004 at the latest.

There are two ways to send the accreditation form. You can print the form out, fill it in, and send it by regular post to:

In you choose this second option, please note that the form can only be sent as an attachment - save it to your computer first, and then fill it in and send it.
